Terms of Limited Scope Representation
We, Ullenberg Law Offices SC, will prepare four estate planning documents (HIPAA Authorization, Healthcare Power of Attorney, Financial Power of Attorney, and Will,) for you based on the information you provide and give you execution instructions. You are our client even if your parent or someone else pays us. Our representation ends when we deliver your documents.
We will not provide ongoing legal advice, court representation, or other services beyond preparing these four documents. We will communicate with you primarily through email.
You must provide accurate information and communicate promptly. If you don’t respond within 7 days, we may end our services. The $395 is due before we start and is non-refundable once we begin.
